摘要:
为了探讨下肢功能性训练对糖尿病足患者下肢骨骼肌质量(lower limb skeletal muscle mass,LSM)、糖脂代谢水平的影响,纳入50~80岁糖尿病足受试者100名,健康受试者50名。将糖尿病足受试者分为下肢功能性训练组(high diabetes group,HDG组)和对照组(high control group,HCG组),健康受试者作为阴性对照组(negative control group,NCG组),以比较下肢功能性训练干预效果。HDG组接受4次/周、60 min/次,为期12周的下肢功能性训练干预,NCG组和HCG组接受常规护理及正常生活。测试干预前后LSM、糖脂代谢指标水平。结果表明:(1)干预后,HDG组LSM均显著上升;(2)干预后,HDG组糖化血红蛋白、空腹血糖、餐后2小时静脉血糖、空腹胰岛素、餐后2小时胰岛素、空腹C肽、餐后2小时C肽水平均显著下降;(3)干预后,HDG组总胆固醇、甘油三酯、低密度脂蛋白胆固醇均显著下降,高密度脂蛋白胆固醇显著上升。得出结论:下肢功能性训练能增加糖尿病足患者下肢骨骼肌质量,改善糖脂代谢水平,从而有效降低糖尿病足发生风险。
Abstract:
Objective:To investigate the effects of lower limb functional training on lower limb skeletal muscle mass (LSM) and glucolipid metabolism level in patients with diabetic foot.Methods:100 subjects with diabetic foot aged 50~80 years and 50 healthy subjects were included.The diabetic foot subjects were divided into a lower limb functional training group (HDG group) and a training positive control group (HCG group),and the healthy subjects served as a negative control group (NCG group) in order to compare the effects of lower limb functional training intervention.The HDG group received the lower limb functional training intervention 4 times/week,60 min/session for 12 weeks,while the NCG and HCG groups received routine care and normal life.The levels of LSM and glycolipid metabolism indexes were tested before and after the intervention.Results:(1) After the intervention,the LSM of HDG group all increased significantly;(2) After the intervention,the levels of glycated hemoglobin,fasting glucose,2-hour postprandial venous glucose,fasting insulin,2-hour postprandial insulin,fasting C-peptide,and 2-hour postprandial C-peptide of HDG group decreased significantly;(3) After the intervention,the levels of total cholesterol,triglyceride,and LDL cholesterol of HDG group decreased significantly,and the level of high-density lipoprotein cholesterol increased significantly,lipoprotein cholesterol increased significantly.Conclusion:After the lower limb functional training,the skeletal muscle mass of the lower limbs of diabetic foot patients increased,and the level of glucose and lipid metabolism was improved.The risk of diabetic foot was effectively reduced.
