研究通过系统综述与Meta分析,评估多种运动干预对老年帕金森病(PD)患者跌倒风险的影响,以期为临床康复实践提供循证依据,助力PD患者改善生活质量。系统检索多个数据库后,共纳入14项符合标准的随机对照试验(RCT),研究对象为60岁及以上的PD患者,干预方式包括平衡训练、抗阻训练、舞蹈训练、跑步机训练及水疗等,干预时长介于4周至24个月之间。Meta分析结果显示,运动干预可显著降低跌倒发生频率、改善跌倒相关风险评估得分,并提升步态稳定性。具体而言,平衡训练和抗阻训练有助于改善姿势控制与下肢肌力;舞蹈训练在提高步态协调性和心理健康水平方面效果显著;结合音乐或听觉提示的跑步机训练可增强步态控制能力;水疗则在改善平衡功能方面具有明显效益。组合干预在多项身体功能指标上表现出更全面的改善。综上,建议在PD患者康复中优先采用组合式运动干预,以综合提升患者生活质量与独立能力。未来研究应扩大样本量并采用更统一的干预方案,以进一步验证其效果及长期效益。
Abstract:
This systematic review and meta-analysis evaluated the impact of various exercise interventions on fall risk among elderly patients with Parkinson’s disease (PD),aiming to provide evidence-based support for clinical rehabilitation practices and improve patients’ quality of life.Through a systematic search of relevant databases,14 randomized controlled trials (RCTs) meeting the inclusion criteria were identified,involving PD patients aged 60 and above.The interventions included balance training,resistance training,dance therapy,treadmill training,and aquatic therapy,with durations ranging from 4 weeks to 24 months.Meta-analysis results indicated that exercise interventions significantly reduced fall frequency,improved fall risk assessment scores,and enhanced gait stability.Specifically,balance and resistance training were effective in improving postural control and lower limb strength,dance therapy significantly enhanced gait coordination and psychological well-being,treadmill training with auditory or musical stimuli improved gait control,and aquatic therapy showed notable effects in improving balance.By comparison,combined interventions demonstrated superior effects in enhancing multiple physical functions.In summary,combined exercise interventions are recommended in PD rehabilitation to comprehensively improve patient quality of life and independence.Future research should include larger sample sizes and standardized protocols to further validate the effects and examine the long-term outcomes.
